Achieve Physical Therapy is contracted with insurance companies in Arizona and Nevada. We offer same day appointments, convenient office hours and accept the following insurances:
If you don’t find your insurance on the list we would be happy to work out an arrangement with you, so you can receive the treatment you need to get back to life!

To our wonderful referring Doctors and their staff who entrust us with the treatment of their patients, we express our gratitude. We want to make it as easy as possible to refer patients to our clinics. If you will just fax us the patient prescription and we will take care of the rest. We will contact the patient to schedule, verify their insurance, and report to you on how they are progressing.
